Abstract
The figures that characterize our Country show a lengthening of the population's average age, in fact life expectancy has a growing forecast trend (Paganelli, 2015). This demographic change, in absence of contrast interventions, would have, as natural consequence, the rise of cared people affected by a pathology in chronic stage as well as of the most serious patients with poly-chronicity. Such a scenario would make, then, the charge on the National Healthcare Service and the system sustainability in the long term more burdensome (Biselli D. 2016).
