Abstract
The ministerial decree number 77 outlines the role of Territorial Operative Centers in coordinating and integrating healthcare services in Italy. The Veneto Region's experience includes 9 COT HUBs, each serving as a link between hospital and primary care, with an emphasis on telemedicine and interprofessional collaboration. This approach aims to ensure seamless care transitions, improve prescriptions appropriateness, and enhance patient-centered care, emphasizing the importance of interprofessional competence.
